Job Description

The Renal Care Coordinator, Outpatient is a non-licensed team member responsible for coordinating non-clinical tasks to support the delivery of case for high risk members with Chronic Kidney Disease (CKD) and End-Stage Renal Disease (ESRD). Responsibilities include, but are not limited to, assisting in care coordination, supporting member education activities, maintenance of member records and track progress, assume liaison role in the facilitation of dialysis appointments and transportation services, assist in monitoring member progress and ensure follow-up on referrals, and identify and address barriers to care such as transportation or non-adherence to treatments.

The goal of the Renal Care Coordinator is to provide support and assistance in the overall care management of CKD and ESRD members on an outpatient basis to ensure the delivery of appropriate linkage of care, prevention of avoidable in-patient admissions and re-admissions, and provision of longitudinal care, including medical, nutritional, environmental, and psycho-social needs.

Education and/or Experience :

  1. High school graduate required.
  2. Minimum 2 years of dialysis experience required.
  3. Minimum of 1 year of experience and knowledge of Healthcare & Managed Care required.
  4. Knowledge of computers, faxes, printers, and all other equipment.
  5. Proficient in MS Office programs (i.e., Word, Excel, & Outlook).
  6. Typing 30 WPM with accuracy.
  7. Ability to deal with responsibility with confidential matters.
  8. Ability to work in a multi-task, high stress environment.
  9. Ability to be flexible with changes in process and procedures.
  10. Critical Thinking
  11. Experience with HMO, Medicare, and or Medi-Cal
  12. Good phone etiquette
  13. Professionalism
  14. Strong analytical skills with the ability to organize, analyze, and disseminate significant amounts of information with attention to detail and accuracy.

Essentials

  1. Assists renal care team with care coordination to ensure that members receive appropriate care by arranging services for CKD and ESRD members, such as dialysis services, home health, DME, physician appointments, specialist appointments, and transportation.
  2. Follows up with patients to ensure arranged services are received.
  3. Provides assistance with member education and support, including, renal treatment options.
  4. Acts as a liaison between patients, healthcare providers, and insurance companies to facilitate communication.
  5. Maintains turnaround time compliance in all aspects of the case management Standards of Operation.
  6. Keeps patient and/or family members or other customers informed of care coordination.
  7. Assists other staff and supports the team approach.
  8. Demonstrates the ability to follow through with requests, sharing of critical information, and getting back to individuals in a timely manner.
  9. Demonstrates honesty and integrity in everyday activities.
  10. Protects privacy for both patients and employees; ensuring all personal health information is kept confidential i.e., (sensitive papers, charts, and reports are not in view of the public).
  11. Recognizes when an error has been made and immediately reports to appropriate manager.
  12. Participates in “service recovery” through follow-up with an upset member, gathering information, and demonstrating empathy.
  13. Treats members and their families with respect and dignity.
  14. Functions as liaison between administration, members, physicians, and other healthcare providers.
  15. Interacts professionally with other healthcare service providers.
  16. Communicates appropriately and clearly to management, co-workers, and physicians.
  17. Consults other departments as appropriate.
  18. Participates in the efficient, effective, and responsible use of resources such as supplies and equipment.
  19. Maintains all reference material that is provided.
  20. Knows and follows the Employee Handbook policies and procedures.
  21. Must maintain a >95% benchmark in quality audits.
  22. Must maintain high productivity based on membership and case load volume.
  23. Maintain a low Unable to Reach/Contact & Declined rate.
  24. Must assist team in case load as needed.
  25. Must not have abandoned calls.
  26. Timely fax server review and clearance.
  27. Attend meetings timely and participate with constructive feedback.
  28. Have a ‘Can Do’ attitude.
  29. Be Proactive in anticipating the needs of the department.
  30. Create an open communication environment by asking questions when in doubt.
  31. Reporting if there are system issues that affect productivity.
  32. Distribution of work: Daily production will vary from day to day. All assigned work must be completed by the end of business day in order to maintain customer service to members and turnaround time compliance.
  33. Protect member’s privacy and adhere to HIPAA regulations at all times.

The pay range for this position at commencement of employment is expected to be between $21 - $23 per hour ; however, base pay offered may vary depending on multiple individualized factors, including market location, job-related knowledge, licensure, skills, and experience.

The total compensation package for this position may also include other elements, including a sign-on bonus and discretionary awards in addition to a full range of medical, financial, and/or other benefits (including 401(k) eligibility and various paid time off benefits, such as vacation, sick time, and parental leave), dependent on the position offered.
