Pediatric LCSW/LMHC Case Manager – Hybrid

HealthCare Support is actively seeking a Pediatric LCSW/LMHC Case Manager – Hybrid to fill an opening with a Fortune 50 Managed Care Company in Hendry or Glades counties.

Daily Responsibilities for Pediatric LCSW/LMHC Case Manager – Hybrid

• Develop, assess, and adjust, as necessary, the care plan and promote desired outcome
• Assess the member's current health status, resource utilization, past and present treatment plan and services, prognosis, short- and long-term goals, treatment and provider options
• Coordinate services between Primary Care Physician (PCP), specialists, medical providers, and non-medical staff as necessary to meet the complete medical socio-economic needs of clients
• Develop plan of care based upon assessment with specific objectives, goals and interventions designed to meet member's needs
• Provide patient and provider education
• Facilitate member access to community-based services
• Monitor referrals made to community-based organizations, medical care and other services to support the members’ overall care management plan
• Actively participate in integrated team care management rounds
• Identify related risk management quality concerns and report these scenarios to the appropriate resources
• Enter and maintain assessments, authorizations, and pertinent clinical information into various medical management systems

• Location: Hendry or Glades counties. A candidate who resides in Clewiston would be ideal, but other areas in those counties such as Labelle or Moore Haven would also work, can stretch to Immokalee if needed
• Travel: Will receive IRS mileage rate - about 50-75% travel. The candidate will have control over when they do their field visits (during business hours) so for example they can complete all their field visits in 1 day and the rest of the week they are WFH. It is infrequent for field visits and are usually just several per week and majority of the time they will be at home.
• This will be a contract role Temp to hire - 6 months but could be extended before conversion to permanent, based on attendance/performance

Required Qualifications for Pediatric LCSW/LMHC Case Manager – Hybrid

• 2+ years of Pediatric clinical experience
• Active LCSW or a LMHC license in FL
• Master’s Degree holder
• Valid and Active Driver’s license
• WFH capability – smart phone, private work space, high speed internet

Preferred Qualifications for Pediatric LCSW/LMHC Case Manager – Hybrid

• Knowledge of healthcare and managed care preferred
• Case management
• MCO
• Bilingual in Spanish highly preferred or Creole
• Experience with EMR

Pay Details: $31.25/hour - $38.50/hour
